HOW TO OPEN, COMPLETE AND SUBMIT
THE FAX CREDIT APPLICATION

By clicking on the link button below, you will open the credit application. You can then type your answers to the questions in the blank fields. When done completing the credit application, right click on any yellow part of the credit application, then click on 'print' in the pop-up menu. Click OK. Your completed application will print. Sign the printed application. FAX it to Ed at: (805) 777-0726.

We MUST have a signed, FAX/Mailed copy from you and these methods are more secure than sending it over the Internet. We respect your privacy and comply with all government regulations concerning the collection, handling and distribution of your personally identifiable information. Security and privacy in all of our transactions is our top priority. We DO NOT recommend sending a credit application by Email.
